Cancellation Policy:

We require at least 24 hours notice to cancel an appointment. If you cancel within 24 hours of your scheduled appointment, you will be billed in full.


New Patients

For your convenience, please download and complete the new patient intake forms and bring them to your first appointment. If you prefer to complete the forms at the clinic, please arrive 20 minutes prior to your appointment time.

First office visits will last approximately 90 minutes while follow-up visits will usually take one hour. Please wear loose-fitting, comfortable clothing and make sure you have had a snack or light meal within two hours prior to your appointment.



Pacific Bridge Wellness is an out-of-network provider. For your convenience, we will provide you with a Superbill that you can submit to your insurance carrier for possible reimbursement. It is solely your responsibility to verify out-of-network benefits with your insurance provider.


Payments is due at the time of treatment. We accept checks and all major credit cards.
