New Patients
Once your appointment is scheduled, you will receive a link to complete the new-patient intake forms. Please be sure to complete the forms prior to arriving for your appointment.
First office visits will last approximately 90 minutes while follow-up visits will usually take one hour. Please wear loose-fitting, comfortable clothing and make sure you have had a snack or light meal within two hours prior to your appointment.

We accept checks and all major credit/debit cards.
In-Office Visits
Current Patients: Payment is due at the time of services rendered.
New Patients: A non-refundable deposit of $95 will be collected at the time of booking with the balance due at the time of services rendered.
Online Video Herbal Consultations
Payment will be due at the time of booking. Herbal formulas will be invoiced separately.
Insurance
Pacific Bridge Wellness is an out-of-network provider. For your convenience, we will provide a Superbill at your request that you can submit to your insurance provider for possible reimbursement. It is solely your responsibility to verify out-of-network benefits with your insurance provider. Pacific Bridge Wellness will not follow-up on any unpaid claims. We do not accept Medicare, Medicaid, or CenCal Health.
