Pacific Bridge Wellness — Membership and Package Terms & Conditions
1. Overview
Pacific Bridge Wellness (“Clinic,” “we,” “us”) offers monthly Membership Plans and Prepaid Packages to provide consistent, high‑quality East Asian Medicine services to our clients. These offerings are managed through Jane.app. By enrolling in a membership or purchasing a package, the patient (“Member,” “Client,” “you”) agrees to the terms below.
2. Services Covered / Exclusions
Memberships and Packages cover treatments only, per Jane.app policy. Products (such as herbal supplements) cannot be included.
All treatments included are acupuncture and East Asian Medicine modalities (e.g., cupping, gua sha, manual therapy, herbal consultation if desired). Herbal supplements and retail goods are billed separately.
Additional services or product charges outside the membership/package (e.g. herbal supplements) will be billed at standard retail rates.
3. 12-Visit Prepaid Package Option
Validity: 12 months from date of purchase
Appointment booking: at your convenience, subject to availability and clinic scheduling.
Redemption & tracking: Visits are applied at checkout at appointment time; remaining sessions and usage appear in the patient’s Jane profile under “Packages & Memberships.”
Transferability: The package is non-transferable — only the patient who purchased it may redeem the sessions.
Refund or credit: Packages are non-reundable. After one year, unused sessions will be converted to credit in patient’s account.
4. Membership Plans (Monthly Recurring)
We offer three membership tiers to suit different patient needs. Memberships provide discounted access to treatments and predictable monthly billing.
Wellness Monthly: Up to 4 visits/month (≈ 1/week)
Healing Intensive: Up to 8 visits/month (≈ 2/week)
Platinum Wellness: Up to 12 visits/month (≈ 3 visits/week)
What is included:
All acupuncture and East Asian Medicine modalities (cupping, gua sha, manual therapy, herbal consultation if desired).
No additional “add-on” fees when appointments are booked under the membership (herbal supplements excluded).
Billing & Payment:
Memberships are billed automatically each month via Jane.app, using a card on file
The invoice is generated per billing cycle; payment is required to maintain membership access.
Usage Rules:
Unused visits do not roll over to the next billing cycle. Each month’s allotment expires at the end of that billing period.
Memberships are non-transferable; each membership applies only to the enrolled patient.
Patients cannot purchase memberships online via the booking portal; all enrollments must be processed by clinic staff in Jane.app.
Enrollment & Management:
Payment method may be updated at any time from the patient’s billing profile.
Memberships may be canceled by clinic staff at any time; clients may not self-cancel via the online booking portal.
Once canceled, the membership ceases at the end of the current billing cycle; unused visits are forfeited.
5. Responsibilities & Limitations
Client’s responsibility: Maintain an active payment method on file; pay invoices on time.
Clinic’s responsibility: Apply correct membership/package status at checkout; track usage via Jane; notify patient of upcoming renewals or expiry if needed.
Legal / Compliance: This agreement covers only services and payments as structured through Jane.app. It does not represent a guarantee of treatment outcomes. Patients acknowledge that herbal supplements, product purchases, and other fees are outside this agreement and are billed separately.
6. Amendments & Notifications
We may revise or update these Terms & Conditions. Any material changes will be communicated to active members at least 30 days in advance. Members retain the right to cancel membership before renewal if they do not agree with updates.
7. Acceptance
By enrolling in a membership or purchasing a package, you confirm that you have read, understood, and agreed to these Terms & Conditions.
